Fat stem cells cure osteoarthritis in clinical trials, doctors report

Vietnam is among the few countries doing clinical trials to treat knee osteoarthritis (OA) using stem cells taken from a patient’s fat tissue, and two of the hospitals in the country doing them say the procedure has proved effective so far. Dr. Bui Hong Thien Khanh, head of orthopedics at the Ho Chi Minh City Medicine and Pharmacy University Hospital, told Vietweek that as of last January his hospital had performed the trials on 21 patients with OA of the knees. Doctors use a needle to harvest about 100cc of patients’ abdominal fat, and extract stem cells from the adipose tissues with a centrifuge. The patients’ blood is extracted and also placed in the centrifuge to get platelet-rich plasma. The stem cells and platelet-rich plasma are then put under a low-energy laser before being injected into the degenerative joints. Khanh said both the plasma and low-energy laser help activate dormant stem cells, which, after activation, can differentiate into cartilage cells to replace the lost ones, and stimulate the damaged cartilage tissues to regenerate. He said reviews six to 18 months after the procedure showed that all the patients had reduced pain, could walk normally, and did not suffer from other complications. An MRI (Magnetic resonance imaging) six months after the operation showed the fibrillation in their cartilage had declined and the damaged cartilage had become thicker, he said. A further 17 have undergone the treatment at his hospital now, he said. Bach Mai Hospital in Hanoi is another hospital to report early success. Dr. Mai Trong Khoa, its deputy director, told Vietweek via phone that nearly 20 OA patients have received the treatment since early last year, and many of them can now walk and sit without difficulty and do not take or have reduced their daily doses of painkillers. Every treatment has Achilles heel OA, also known as degenerative joint disease, is a condition in which the cartilage cushioning the joint surface and protecting two bones from rubbing against each other wears out at a faster pace than the body can repair. The most common of the various kinds of arthritis, it causes pain, limits a person’s movement, and can even lead to disability, doctors warned. It is usually thought to be connected with aging, but doctors said that many young Vietnamese are now getting it. According to the Vietnam Rheumatology Association, 60 percent of people around 65 and 85 percent of those around 80 have OA, but so do a third of all people around 35. The doctors said overuse of joints, trauma caused by traffic or sports accidents, obesity, and inactivity also cause the condition. Depending on the seriousness of the condition, doctors prescribe medication, arthroscopic surgery (to remove damaged cartilage tissue, drill into bone to stimulate the marrow to create new cartilage tissue, or transplant a cartilage), or joint replacement. But they said use of painkillers can lead to side effects like stomach pain, while arthroscopic surgery offers no guarantee against the recurrence of OA. A joint replacement, which is normally done in the most severe cases when lots of cartilage tissues are lost, the joint is deformed, subchondral bones are exposed, and the bones are misaligned, needs to be done again after 10-15 years, they said. Khoa of Bach Mai said: “If the disease is not treated, it will affect the patients’ quality of life. “Using autologous [derived from patient] stem cells offer another option for knee OA treatment.” The procedure is easy to perform, and safe and effective since it rarely causes complications and the body does not reject the tissues unlike when donor tissues are used. Khanh of the Ho Chi Minh City Medicine and Pharmacy University Hospital said stem cells harvested from fat tissues are plentiful and easier to obtain than from bone marrow. The bone marrow yields only a few stem cells and they need to be cultured for more than two weeks under specific conditions, he said. The marrow is taken from the pelvis and the process is very painful, he said. But Khoa said the procedure cannot be done on all patients since it depends on several factors like age and health condition. Nguyen Van Kinh, an advisor at Bach Mai’s Vietnam Gene Therapy Center, said in the most severe (grade IV) cases, it only helps the cartilage tissues develop and release a fluid to improve the knee joints’ lubrication and relieve pain, and cannot return the knee to its original structure. The use of stem cells in general and to treat knee OA is still undergoing clinical trials also in Australia, Canada, Singapore, South Korea, the US, and a few other countries. The health ministry has also allowed People’s Hospital 115 and Van Hanh General Hospital in HCMC to carry out trials for two years starting April. Stem Cells May Be Key to Multiple Sclerosis Treatment

New research released in the journal Stem Cell Reports suggests that stem cell therapy may be the key to treating patients with multiple sclerosis (MS). Currently, there are few medically-accepted treatments for MS, and these only help with symptoms in the early stages of the progressive neurological disorder. The new research, however, shows that stem cells may actually help in reversing the progression of this disease. Multiple sclerosis is thought to be an auto-immune disease in which the bodies immune system attacks the myelin sheath around nerve fibres. This myelin, a fatty substance that is vital for proper nerve function, also protects the nerves, but does not readily grow back following loss. As a result of this progressive demyelination in MS, patients begin to lose sensory abilities, such as physical sensation and vision, while experiencing other problems such as difficulty walking fatigue and pain. Using a mouse model of MS, in which animals had been provided with a virus known to destroy the myelin within the spinal cord, the research team found that stem cell injections were able to reduce the motor problems exhibited by these animals and gave them the ability to walk within just two weeks. This amazing recovery was likely due to the changes in the immune system that the scientists were able to note, such as reduced inflammation in the central nervous system and even a regrowth of myelin in the spinal cord that was seen six months following treatment. What interested the researchers was that these beneficial effects occurred despite the complete rejection of the stem cells after only one week following the initial injection. The authors believe that this rapid and sustained recovery was initiated by specific proteins released by the stem cells. These proteins are known to modulate the activity of the immune system, and were found to be highly expressed by the stem cells. To support this idea, some mice were given antibodies to prevent these proteins from working, and this prevented their immune system from responding to the therapy. Currently, there is no known cause of MS, and available treatments only affect the early stages of this disease. Antiviral treatment is the most common, and helps reduce the frequency of attacks, but ultimately does not halt the progression of the disease. Given the lack of adequate medicine, many patients suffering from MS have experimented with alternative medicines, including medical marijuana, yoga, acupuncture and other dietary supplements. One more controversial therapy is known as “liberation treatment.” Pioneered by Dr. Zamboni, this treatment surgically widened the veins in the neck, but recent research has suggested that the theory behind this procedure is lacking any evidence. Over 2.3 million people world-wide suffer from MS, with the highest rates of this disease occurring in northern Europe and North America. It is believed that approximately 400,000 Americans are affected, while 100,000 Canadians suffer from MS, giving Canada the dubious distinction of having the highest rate of MS in the world. If the current study can be replicated, the possibility of forthcoming human trials arises, and for many, not a moment too soon. Abstract Stromal vascular fraction (SVF), containing large amount of stem cells and other regenerative cells, can be easily obtained from loose connective tissue that is associated with adipose tissue. Here we evaluated safety and clinical efficacy of freshly isolated autologous SVF cells in a case control study in patients with grade 2-4 degenerative osteoarthritis (OA). A total of 1128 patients underwent standard liposuction under local anesthesia and SVF cells were isolated and prepared for application into 1-4 large joints. A total of 1856 joints, mainly knee and hip joints, were treated with a single dose of SVF cells. 1114 patients were followed for 12.1-54.3 months (median 17.2 months) for safety and efficacy. Modified KOOS/HOOS Clinical Score was used to evaluate clinical effect and was based on pain, non-steroid analgesic usage, limping, extent of joint movement, and stiffness evaluation before and at 3, 6, and 12 months after the treatment. No serious side effects, systemic infection or cancer was associated with SVF cell therapy. Most patients gradually improved 3-12 months after the treatment. At least 75% Score improvement was noticed in 63% of patients and at least 50% Score improvement was documented in 91% of patients 12 months after SVF cell therapy. Obesity and higher grade of OA were associated with slower healing. In conclusion, here we report a novel and promising treatment approach for patients with degenerative OA that is safe, cost-effective, and relying only on autologous cells.

A Landmark Stem Cell Therapy Shows Striking Results Against Aging

Two clinical trials have shown that Mesenchymal stem cells (MSCs) are safe and highly effective in treating age-related frailty. Buoyed by the success of their initial trials, scientists from the University of Miami’s Miller School of Medicine are now moving their research into larger trials. Once these steps have been completed, the stem cell-based technology could be the first anti-aging treatment to get the approval from the FDA. Fact: everyone gets old. Another fact: many diseases and conditions appear as people grow old. Among the many unwanted signs of aging, perhaps one of the most common (if not the most common) is frailty, an age-related health problem often believed to be an inevitable part of ageing. On October 12, two clinical trials were published online in The Journals of Gerontology. The reports support a longstanding theory about age-related frailty being caused by stem cell depletion. More importantly, the trials also offered a new way of treatment for sufferers of age-associated frailty. Two teams from the Interdisciplinary Stem Cell Institute (ISCI) of the University of Miami’s Miller School of Medicine, conducted their respective clinical trials to study the safety and effectiveness of ‘Mesenchymal’ stem cells (MSCs) in treating age-related frailty, a condition that encompasses a range of symptoms including fatigue, slower movements, muscle weakness and unintentional weight loss. Speaking in simple technical terms, stem cells are basically “starting” cells that can morph into any type of cell in the body. There are two types of stem cells: embryonic stem cells and adult stem cells. Among the different subtypes of adult stem cells, it is the ‘Mesenchymal’ stem cells (MSCs), a diverse subset of self-renewal multipotent precursors present in many adult tissues and currently used in the fields of regenerative medicine and tissue engineering, that are drawing intense interest in the world of science. MSCs are used to treat many conditions, including cancer and heart disease. ISCI researchers conducted two human trials. The Phase 1 trial involved 15 frail patients whose average age was 76 years. These patients were given a single infusion of MSCs which were sourced from bone marrows of younger donors whose ages ranged from 20 to 45 years old. Six months later, strikingly effective results appeared when it came to tackling key factors in aging, as every patient demonstrated improved fitness conditions, gained a better quality of life, and showed improved tumor necrosis levels. On the other hand, Phase 2 was a randomized, double-blind trial with a placebo group in the mix. The results were similar to those of phase 1—adverse effects were absent and improvements were seen in patients. Frailty is characterized by the age-related decline in a person’s strength, endurance, physical and psychological functions. This condition often leads to hospitalizations, dependency and, worse, death. Few techniques are available to manage age-associated frailty (like exercise, nutritional supplements, and other multidisciplinary techniques) but none so far has gained the formal approval of FDA. As age-related frailty is believed to be a product of stem cell depletion, it appears that a stem cell-based treatment is the most conspicuous way of countering it. Frailty affects around 10 to 11% of the world’s senior population, and MSC will surely bring benefits (also comfort and happiness) to this group. “With the aging of the population, stem cells hold great promise to treat aging-related disability and frailty, improving physical capacity and quality of life,” said Joshua M. Hare, ISCI Director and one of the researchers involved in both trials. This is not the first time though that bone marrow-derived human MSCs are studied for their anti-aging effects. Previous studies have shown that these cells act on injury sites, have anti-inflammatory and antifibrotic properties, and also have pro-regenerative powers. As such, MSC treatment not only provides a way for treating frailty but also offers hope for people who suffer from a wide range of inflammatory and degenerative diseases. In other words, MSCs offer a multifactorial approach to addressing several aspects of the disease called aging. As for the research iteslf, scientists of Miami’s Miller School of Medicine are now moving into phase 2b, this time with 120 patients selected across ten locations. After this, an even larger, randomized trial will be launched as phase 3. This will be the last step before seeking regulatory approval for the treatment’s public use. With the remarkable results efficacy-wise posted by phase 1 and 2 trials, MSCs landmark stem cell treatment is on track to become the first anti-aging remedy to get the nod from the FDA.
